ANPs Out of hours required
We require a number of experienced Advanced Nurse Practitioners for evening, weekend and bank holiday cover to improve access for patients to the Out of Hours services and facilitate a clear patient pathway through primary, community, secondary and tertiary care where required. Ideally candidates should have extensive experience working in Primary Care.
You will undertake assessment of presenting complaints and offer advice, health education, treatment and referral, as appropriate, including the prescription of medicines where clinically required. Therefore a non-medical prescribing qualification is essential.
You will need to be able to work independently without further clinical presence, managing autonomous case loads alongside the General Practitioners. You will need a broad experience of using advanced clinical examination and assessment skills within your practice. Candidates will be required to see children through to geriatrics within your competencies safely.
Shifts available are home visits, face to face surgery shifts, or telephone triage.
The Out of Hours sites currently available to work in are Pendle , Rossendale, Burnley and Blackburn.
Working pattern: Either contractual weekly shifts or flexible ad hoc to service needs

The Acute Visiting Service at East Lancs Medical Service provides a high quality, responsive and clinically safe service to patients at risk of hospital attendance and admission. It is a GP led service that is supported by advanced nurse practitioners. The hours of work are Monday – Friday from 9am-7pm.
We are looking to appointment an advanced nurse practitioner that is able to start in October 2017, (As we envisage most candidates may need to give 3mths notice in their current role.)
The service provides rapid in-hours assessment for patients at risk of a non-elective admission with the aim of keeping the patients in their homes or place of residence. With a focus on adults, who are generally over 65 and have a genuine risk of hospital admission or A&E attendance. We carry out these home visits on behalf of the Blackburn with Darwen Gp practices.
There will also be an element of telephone triage at times, and carrying out audit work.
Using your independent prescribing skills, Candidates are expected to work autonomously and be confident working without immediate further clinical presence. They should ideally be familiar with EMIS web , adastra , and be IT literate. The practitioner will have the opportunity to develop the service , and will take part in audit , and feedback concerns to refine the service.

BAND 7 EQUIVALENT £31,00- £41,000 - NURSE PRACTITIONER ROLE – FULL TIME ,
We are looking for a motivated, enthusiastic, prescribing Nurse Practitioner to enhance our current team. This is an exciting time to Join ELMS as you will be helping to roll out a new acute patient assessment service. The service will evolve over time and you will help to shape the direction of the service. Trainee nurse practitioners will be considered (if they have already achieved the v300 prescribing qualification.) You will prioritise the calls and direct patients or health professionals to the most appropriate service, trying to avoid inappropriate hospital admissions, keeping patients at home where possible.
On behalf of the service you will carry out complex clinical triage in a sensitive manner providing assessment, advice and information in line with clinical assessment software whilst utilising professional judgement and referring to other agencies/ services where appropriate.
Hours of work will be Monday – Friday initially, and you will be based at East Lancs medical services headquarters, Blackburn. We welcome informal visits, we are a friendly bunch!
